    Your $55 annual fee will be credited back to your account if $24,000 or more in net purchases posts to your company's account within your 12-month annual statement year. You may add additional cards to your account for $10/year.13

2 U.S. Bank will credit your FlexPerks Business Travel Rewards Visa card account with FlexPoints as follows. For Net Purchases, earn one FlexPoint for every $1. FlexPoints will be awarded as long as your Account is open and not 5 days or more past due at the close of your Visa billing period. We will not award FlexPoints for Advances, Convenience Checks, Balance Transfers, Finance Charges and Fees, credit insurance charges, transactions to fund certain prepaid card products, U.S. Mint purchases, or transactions to purchase cash convertible items. We reserve the right to adjust the number of FlexPoints for Purchases or to stop issuing FlexPoints for Purchases on the Account, upon notice to you. FlexPoints will expire five years from the end of the calendar quarter in which they were earned. FlexPoints are redeemed or expired in the order in which they were earned and awarded to the account.

3 FlexPerks Business Travel Rewards Visa cardmembers may earn additional FlexPoints for purchases at merchant locations in the following categories: airline, gas or office supplies (each, a "Category"). You will earn FlexPoints at a rate of two FlexPoints in addition to every $1 spent in the one Category in any given monthly billing cycle that has the highest total of Net Purchases charged to your Account (the "Highest Category").

5 FlexPerks Travel Rewards Visa Signature and FlexPerks Business Travel Rewards Visa cardmembers will earn FlexPoints at a rate of three (3) FlexPoints per every $1 in Net Purchases and FlexPerks Select Rewards Visa cardmembers will earn FlexPoints at a rate of 1.5 FlexPoints per every $1 in Net Purchases during the current month's billing cycle for any merchant location that classifies itself as a Charitable and Social Service Organization. U.S Bank does not have the ability to control how a merchant chooses to classify their business and therefore reserves the right to determine which purchases qualify for additional FlexPoints. Bonus FlexPoints will be awarded within 60 days of donation.

6 FlexPerks Travel Rewards Visa Signature cardmembers may receive up to a $25 airline allowance with each redeemed airline award travel ticket. Cardmembers who redeem FlexPoints for award travel may be credited up to $25 on a future statement for Qualified Purchases made using their FlexPerks Travel Rewards Visa Signature card with the airline carrier providing award travel flight. "Qualified Purchases" are any purchases made with your FlexPerks Travel Rewards Visa Signature card that post to your account under the airline carrier providing your award travel flight between the dates of your award travel flight. Cardmembers must call Cardmember Services at 877-978-7446 to request a statement credit within 90 days of a Qualified Purchase. Once the request is approved by Cardmember Services, the cardmember will receive a statement credit within 4 to 6 weeks for the amount of the qualified purchase, up to $25 per award ticket.

U.S.  Bank does not have the ability to control how a merchant chooses to classify their business and therefore reserves the right to determine which purchases qualify for allowance. You may redeem allowance for Qualified Purchases only if the outstanding balance of your account is below its revolve limit and if your account is open and in good standing under the terms of your Cardmember Agreement. Once you or we close your account for any reason, your allowance may be forfeited.

7 You will have the ability to redeem FlexPoints for award travel on more than 150 airlines. FlexPerks Cardmembers may redeem if their accounts are open and in good standing under the terms of their Cardmember Agreement. All FlexPoints redemptions are final. FlexPoints are not transferable, are not redeemable for cash and cannot be used as payment for obligations owed to U.S. Bank or its affiliates. There are no refunds, exchanges, replacements or conversions for currency, credit, other points or rewards for no-shows or unused portions or rewards. Determination and payment of tax liability is the sole responsibility of the cardmember. We reserve the right to verify and adjust FlexPoints at any time before or after posting any redemption. Once you or we close your account for any reason, all accumulated FlexPoints may be forfeited. Cardholders must have a Frequent Flyer number with the airline if they wish to earn Frequent Flyer miles on a wand ticket. 

Tickets are subject to the applicable airline rules and restrictions. For Internet reservations, if an airline does not offer e-ticketing or if you request a paper ticket, there will be a minimum shipping fee assessed of $29.95 per transaction. For phone reservations, if paper tickets are requested when e-tickets are available, a minimum $50 per ticket service fee will apply. A minimum $15 shipping/handling fee will apply for all paper tickets.

12 This Card provides the benefit of a No Pre-Set Spending Limit and a Revolve Limit that allows you to carry a designated part of your balance from one statement to the next. A No Pre-Set Spending Limit does not mean unlimited spending.  Authorizations are provided using information such as account history, credit record, income verification and liquid assets. If card issuer authorizes a transaction that causes you to exceed your revolving credit line, then you must pay, as a part of your monthly minimum amount due, the amount by which your new balance exceeds your credit line.

13 The FlexPerks Business Travel Rewards Visa card annual fee is $55 per year for Account Owners and $10 per year for Authorized Employees. FlexPerks Business Travel Rewards Visa cardmembers who spend $24,000 or more in Net Purchases that post to the Company's account within their 12-month annual statement year will have their $55 Account Owner annual fee and each additional $10 Authorized Employee annual fee credited back to their account.
