You can request up to two additional cards1 for family members at no cost. You'll enjoy the peace of mind that comes with knowing your loved one has access to credit when they need it.

To request, simply complete and mail or fax the Authorized User Form (PDF).2 

To add an individual to your Account as a joint owner, simply complete and mail or fax the Joint Owner Form (PDF).2
While logged into Online Banking, you can request a copy of your Cardmember Agreement.
While logged into Online Banking, you can request a Convenience Check.
While logged into OnlineBanking, you can request to Transfer a Balance.
While logged into Internet Banking, you can change your Overdraft Protection preferences on your U.S. Bank Checking account.

The Authorized Officer is authorized by the Company to execute binding agreements on the Company’s behalf and is required to be a Cardmember. The Authorized Officer and the Authorized Representatives are the only person(s) who can make changes to the account(s) such as adding, changing, and/or deleting Cardmembers. To set up, simply complete and mail or fax the Authorized Officer Form (PDF).2
Authorized Representatives are authorized to make modifications to existing cardholder account(s), change credit limits, and add new cardholders for the Company named below. To set up, simply complete and mail or fax the Authorized Representative Form (PDF).2
You can elect to open new Employee Accounts, change existing Employee credit limits or close Employee Accounts.  To make changes, simply complete and mail or fax the Employee Change Form (PDF).2
Minimize your company's risk, while giving your employees the freedom to use their business credit cards with limits that you set. Visa Payment Controls is a free service you can use to individually customize each of your employee's business credit cards to control where, when and how your employees use them.

It's easy to set controls that limit individual card use by:

  • Time of day or day of week
  • Geographical area - both domestic and international
  • Merchant type - includes the ability to allow "gas only" purchases
  • Dollar amount
  • Transaction types - such as cash advances or cash withdrawals

U.S. Bank may change APRs, fees, and other Account terms in the future based on your experience with U.S. Bank National Association and its affiliates as provided under the Cardmember Agreement and applicable law.

Business Cardmembers: U.S. Bank will increase your rate to a Penalty Rate if you:  1) Make payments 5 calendar days late twice, or 30 calendar days late once. 2) Make a payment that is returned; or 3) If you exceed your Credit Limit 2 times in 12 consecutive months.

The creditor and issuer of these cards is U.S. Bank National Association, pursuant to separate licenses from Visa U.S.A. Inc. and American Express. American Express is a federally registered service mark of American Express.
